A date, a reason and a forwarding address. Submitting here creates a dated record rather than a text message somebody may not remember.
The forwarding address matters more than it looks: it is where your deposit is sent.

The deposit has a clock on it
Once you hand back the keys, your landlord has a set number of days to return the deposit with an itemised list of any deductions. The number comes from your state.
